4. How do I register a birth?
In the case of births that occur in the City of Vaughan,
all relevant documents are assembled, recorded and forwarded to
the Registrar
General's office. There is currently no charge for this service.
As the City does not have a hospital within its municipal boundaries,
we are only registering home births. If your child was born at York
Central Hospital, for example, you need to register the child's
birth with the Town of Richmond Hill.
The City needs two documents to register a birth - Form 1 Notice
of Live Birth and Form 2 Statement of Live Birth. Form 1 is completed
by the medical practitioner/midwife and forwarded to the City Clerk's
Department as is Form 2, which is completed by the parent(s). The
two forms are matched, reviewed and sent to the Province.
