ADULT ENTERTAINMENT ATTENDANT:

This licence is required to either provide services designed to appeal to erotic or sexual appetites or inclinations or to operate an establishment where these activities take place

  1. Completed application form
  2. Fee
  3. Proof of being at least 18 years of age
  4. Proof of being eligible to gain employment in Canada such as a Canadian birth certificate, citizenship card, valid work permit or a Picture I.D.Ontario Health card
  5. Completed administrative monetary penalty clearance form

This licence is required to either provide services designed to appeal to erotic or sexual appetites or inclinations or to operate an establishment where these activities take place

BANQUET HALL:

This License is required for any building or hall used for the entertainment of a large group of people where food or alcohol may be provided

BILLIARD HALL:

This licence is required to operate any facility where one or more billiard tables or pool tables or other games of a similar nature such as table soccer or table hockey.

  4. Building / Zoning Clearance
  5. Completed administrative monetary penalty clearance form

BODY RUB ATTENDANT:

This licence is required to either perform massages or body rubs by any person not licensed or registered to perform massages for therapeutic purposes (such as a Registered Massage Therapist) or to operate an establishment where those take place

  1. Completed application form
  2. Fee
  3. Proof of being at least 18 years of age
  4. Proof of being eligible to gain employment in Canada such as a Canadian birth certificate, citizenship card or valid work permit
  5. Police Clearance Letter
  6. Employment letter from a licensed parlour owner
  7. Completed medical clearance form (no older than 90 days from date of issuance)

BODY RUB OPERATOR:

This licence is required to operate a premises where massages or body rubs are performed as part of a business by any person not licensed or registered to perform massages for therapeutic purposes (such as a Registered Massage Therapist)

This License is required to install Charity Clothing donation drop boxes on private property within the City of Vaughan

  4. List and sketch of each proposed Charitable Clothing Drop Box location
  5. Certificate of Insurance ($2,000 000.00)

DRIVING SCHOOL (Classroom):

This licence is required to operate any place where a person teaches another to drive or operate a car or any other motor vehicle

  1. Completed application form

  3. Business Registration Documents, such as Articles of Incorporation or Master Business License
  4. Building / Zoning Clearance
  5. Completed administrative monetary penalty clearance form

Driving School Instructor with Vehicle:

This licence is required to teach others to drive any vehicle

  3. Business registration papers (if applicable)
  4. Safety standards certificate (no older than 90 days from date of issuance)
  5. Proof of insurance ($2,000,000 Liability)
  6. Vehicle ownership
  7. Valid Ontario Class "G" driver's license
  8. Driver's Abstract (no older than 90 days from date of issuance)
  9. Police clearance Letter (no older than 90 days from date of issuance)
  10. Proof of being at least 18 years of age
  11. Proof of being eligible to gain employment in Canada such as a Canadian birth certificate, citizenship card or valid work permit
  12. Valid Ontario Driving Instructor's license
  13. Vehicle inspection
  14. Completed administrative monetary penalty clearance form

Driving School Instructor without Vehicle:

This licence is required to teach others to drive any vehicle

EATING ESTABLISHMENT:

This licence is required to operate a business where food and drink are prepared and offered for sale

FOODSTUFF:

MUNICIPAL LIQUOR LICENSE CLEARANCE

This clearance is required if you will be serving alcohol as part of your business.
